Please note....
All NEW clients are required to complete an intake form and short medical history questionnaire PRIOR to their first appointment.

Once your first appointment is booked - you will receive a confirmation email/text which will include a link to the forms.  These forms can easily be completed online.

If you don't have access or are unable to complete your forms online - not to worry!  We will be happy to assist you in-person or over the phone prior to your first appointment!
